At St Louis, our Restorative Care Pathway offers a structured, person-centred, and time-limited program designed to help older South Australians bounce back after illness, injury, or reduced activity. Delivered by our multidisciplinary team, this pathway supports you to regain function, improve wellbeing, and stay independent — all from the comfort of your home.


What makes it different?

  • Tailored to you: Your goals, preferences, and cultural background shape your care.
  • Short-term, high-impact: Most programs run for 6–12 weeks, with the option to extend to 16 weeks if needed.
  • Team-based care: Allied health, nursing, and care staff work together to support your recovery.
  • Goal-driven: Every plan is built around measurable outcomes and reviewed regularly.
  • Smooth transitions: We help you move to ongoing services or discharge with the right supports in place.

 

How it works


1. Referral & Eligibility


You can be referred by your GP, hospital discharge planner, family member or St Louis staff.


You will be appointed a Restorative Care Partner who will confirms eligibility, helps you sign the service agreement, and schedules your first visit.


2. Goal Setting


Your Restorative Care Partner co-create your care plan through collaborative discussions with you and your support network.
Your goals guide everything we do.


3. Tailored Care & Risk-Informed Decisions


Your plan includes time-limited interventions (typically 8–12 weeks) delivered by qualified clinicians.


4. Service Delivery


Care is delivered in your home or community, coordinated by your Restorative Care Partner.


Services may include:

  • Allied Health: Physiotherapy, Occupational Therapy, Dietetics, Speech Pathology, Nursing
  • Personal Care: Hygiene, grooming
  • Domestic Help: Cleaning, meal prep
  • Social Support: Transport, companionship
  • Equipment & Modifications: Mobility aids, assistive tech, home modifications

5. Weekly Monitoring & Review


Your progress is reviewed weekly by our multidisciplinary team.
We adjust your plan as needed — including extending timeframes or applying for extra funding.


6. Exit Planning


We begin planning your transition several weeks before completion.
Depending on your progress:

  • Goals achieved → Discharge
  • Partial progress → Transition to ongoing services
  • Escalation → Referral to higher-level care
7. Evaluation & Feedback


We collect your feedback and conduct team debriefs to improve future care.

 

Budget & Value

Our restorative care pathway is cost-effective and high-impact, with a typical budget of around $6,000.
Funding covers:

  • Group programs (e.g., exercise, cognitive stimulation)
  • Equipment & materials (e.g., hydration tools, memory aids)
  • Home modifications
  • Meal services & supplements
  • Client education & support
  • Interpreter or cultural liaison services
  • Transport assistance
  • Program management & quality improvement (up to 10% of budget)


Restorative Care Pathway: High Falls Risk

This pathway supports older adults at high risk of falls with targeted therapy, home modifications, and education.

Our goal is to reduce fall-related harm, improve mobility, and restore confidence in daily life.

Key supports include:

  • Physiotherapy & balance training
  • Home safety assessments & modifications
  • Mobility aids & assistive technology
  • Education for clients and carers

 

Restorative Care Pathway: Frailty, Sarcopenia & Multimorbidity

Strengthening your body, your health, and your independence.


Designed for clients experiencing frailty, muscle loss, or multiple chronic conditions, this pathway focuses on improving physical function, reducing hospitalisation risk, and supporting long-term wellbeing.

Key supports include:

  • Allied health therapy (physio, dietetics, OT)
  • Chronic disease management
  • Nutritional support & supplements
  • Personalised exercise programs

 

Restorative Care Pathway: Living with Dementia

Supporting independence, dignity, and quality of life.


This pathway helps people living with dementia remain safely at home through a strengths-based, person-centred approach. We focus on enhancing daily function, reducing risks, and supporting carers.

Key supports include:

  • Cognitive stimulation & memory aids
  • Home modifications for safety
  • Carer education & respite
  • Multidisciplinary dementia care

 

Restorative Care Pathway: Mental Wellbeing & Social Connection

Bringing connection, purpose, and joy back into everyday life.


This pathway supports individuals experiencing loneliness, low mood, or social isolation. We help clients reconnect with their community and rediscover meaning through tailored engagement.

Key supports include:

  • Social support & companionship
  • Transport to community activities
  • Mental wellbeing coaching
  • Group programs & peer connection

 

Restorative Care Pathway: Nutritional Support

Fueling recovery, strength, and independence.


Nutrition plays a vital role in recovery and wellbeing. This pathway helps clients improve their nutritional status to better manage chronic conditions and maintain physical and cognitive function.

Key supports include:

  • Dietitian-led assessments & plans
  • Meal delivery & supplements
  • Education on hydration & nutrition
  • Modified utensils & eating aids

 

Restorative Care Pathway: Swallowing Difficulties (Dysphagia)

Safe, dignified, and enjoyable eating — every day.


This pathway supports individuals with swallowing difficulties to eat and drink safely, prevent complications, and maintain quality of life.

Key supports include:

  • Speech pathology & swallowing therapy
  • Modified diets & texture support
  • Education for carers & families
