Our Clinical Services – Supporting You to Live Safely at Home

At St Louis Home Care, our clinical services are designed around your choices, preferences, and individual goals. We believe you should be at the centre of all decisions about your care. Our warm, professional nursing team works alongside you—and your family if you wish—to help you live safely, comfortably, and as independently as possible in your own home.

We offer flexible support that adapts to your needs, from health check-ups and medication assistance to wound care, diabetes management, dementia support, and specialised nursing care. Together, we will create a care plan that reflects your wishes, ensuring you receive the right support to stay healthy and enjoy life at home.

Comprehensive Clinical Assessment

A Comprehensive Clinical Assessment is an important step to understand your health and preferences. Conducted by a Registered Nurse in your home, it covers:

• Skin health – checking for any issues with your skin.
• Risk of falls – assessing your balance and mobility, with strategies to help prevent falls.
• Memory and thinking – reviewing cognitive health.
• Nutrition – ensuring you’re eating well and maintaining a healthy weight.
• Bladder and bowel health (Continence) – managing any continence needs respectfully and discreetly.

After the assessment, we’ll work with you to update or develop a care plan based on your goals and preferences. We will review your health regularly—every six to twelve months, or sooner after any major change such as a hospital stay—to make sure your plan always reflects your needs and wishes.

Medication Management

Medication management is a shared process—we work in partnership with you to ensure your medicines are taken safely, confidently, and in a way that works for you.

Our Registered Nurses will review your medications in consultation with your GP and pharmacist. If your medicines change, for example after a hospital visit, we’ll update your plan so everything stays correct.

Once your plan is in place, you can choose the level of support that suits you—whether that’s trained care workers prompting you to take your medication or administering it for you. This flexible service reduces stress, prevents mistakes, and supports your independence.

Wound Assessment and Management

If you have a wound, sore, or skin ulcer, our nursing team will provide care that respects your comfort, privacy, and preferences.

A Registered Nurse will assess your wound, create a treatment plan tailored to your needs, and provide ongoing support. We will visit as often as required until the wound heals, or for long-term management if needed.

Your wishes guide the approach—we’ll explain each step, involve you in decisions, and adapt to what feels most comfortable for you.

Diabetes Management

Living with diabetes can be complex, but you remain in control. Our nurses will partner with you to develop a care plan that supports your lifestyle, preferences, and goals.

We can assist with:

• Monitoring your blood sugar levels
• Reviewing your medications or insulin management
• Providing practical advice on diet and nutrition
• Supporting foot health and preventing complications

Your plan will be reviewed regularly (every six months or sooner) to ensure it continues to meet your needs and wishes.

Catheter Care

If you have a urinary catheter, we will provide discreet, respectful care that supports your dignity and comfort.

Our nurses will assess your catheter care needs, provide education to you and your carers, and schedule regular reviews. You will always be informed and involved in any changes to your care.

Specialised Nursing Services

In addition to the above, we offer tailored nursing support for specific needs, including:

• Stoma care
• Continence management
• Pain and symptom management
• Support with other chronic conditions (heart, lung, arthritis, etc.)
