Palliative Care – End of Life Care
The Support at Home Program has dedicated funding and services to support you to remain at home in your final months, if that is your wish.
The End-of-Life Care pathway offers up to roughly 3 months of palliative care support (up to ~12–16 weeks) with an additional budget (approximately $25,000 for the period). It can fund things like specialist nursing, personal care, respite for family, and comfort measures for quality of life.
This pathway is in addition to the specialist Palliative Care Services provided through the State funded Palliative Care Networks.
