Dementia Support Planning
Home Care Package support for people living with Dementia
Caring for someone with dementia requires a deep understanding of their preferences, routines and values. As part of your Home Care Package, we can offer support and services for people living with Dementia to live safely and with comfort in their own home.
Our Registered Nurses will work with you and your family to create a Dementia Support Plan that honours the person’s history, personality and comfort. This plan helps everyone involved in care from care workers and you as family caregivers to respond consistently, reduce distress and promote a calm environment.
We review the plan every six months—or sooner if things change—ensuring support evolves alongside the person’s needs and preferences.
